The Revolutionary Invention of Dr. Willem Kolff: The Man Who Built the First Artificial Kidney

Before the 1940s, a diagnosis of total kidney failure was an absolute death sentence. Without a way to filter toxins from the blood, patients succumbed to uremic poisoning. That insurmountable medical barrier was shattered during the height of World War II by a young Dutch physician named Dr. Willem “Pim” Kolff, who improvised everyday materials into the world’s first working artificial kidney.

Ingenuity Amid Scarcity

When Nazi Germany occupied the Netherlands in 1940, Dr. Kolff relocated to the municipal hospital in the small town of Kampen to continue his practice and avoid serving under a regime-appointed administrator. Operating under extreme wartime scarcity—where materials were tightly restricted and manufacturing resources were heavily guarded—Kolff refused to abandon his vision of building a device that could wash toxins from human blood.

Working alongside local engineer Hendrik Berk, Kolff constructed his historic rotating drum dialyzer in 1943 using a remarkably unconventional array of spare parts:

  • Cellulose Sausage Casing: Used as a semi-permeable membrane to filter the blood, sourced from hot dog casings.
  • Washing Machine Parts & Water Pumps: Utilized to manage fluid movement, drawing mechanics from a Ford Model T cooling system.
  • A-19 Airplane Parts: Built using aluminum framing salvaged from a shot-down German bomber.
  • A Sewing Machine Motor: Adapted to rotate the wooden drum holding the long, coiled membrane.

From Desperation to Triumph

The early road was heartbreaking. Kolff tested his device on sixteen consecutive patients with kidney failure, all of whom tragically passed away as he refined the mechanics and dosing.

Despite profound discouragement, Kolff persevered. On September 11, 1945—shortly after the liberation of the Netherlands—his persistence finally paid off. He successfully dialyzed a 68-year-old woman suffering from a uremic coma over an 11-hour session. She became the world’s first long-term survivor of hemodialysis, proving once and for all that mechanical blood filtration was entirely possible.

A Legacy of Saving Millions

Following the war, Kolff selflessly mailed his blueprints and distributed his early drum dialyzers to medical centers worldwide, laying the groundwork for modern dialysis. In 1950, he immigrated to the United States to join the Cleveland Clinic, where he later helped develop disposable twin-coil dialyzers and expanded his pioneering work into artificial hearts and heart-lung machines.

Though he was nominated multiple times for the Nobel Prize, Dr. Kolff’s true reward was the countless lives saved. Recognized universally as the “father of artificial organs,” his wartime improvisation in a small Dutch hospital transformed modern medicine and continues to sustain millions of lives today.
